PRESCRIPTION MONITORING PROGRAM WEBSITE.
THIS INFORMATION MAY BE VIEWED BY AUTHORIZED PERSONNEL.

FINANCIAL POLICY:
Co-pays, deductibles and co-insurance are due at time of service.
A current copy of your insurance card must be presented at each visit. It is your responsibility to provide us with the most current insurance information.
We bill your insurance as a courtesy to you. It is your responsibility to know and understand your benefits of your insurance. It is your responsibility to promptly pay any balance that your insurance does not pay.
If you are unable to pay your bill upon receipt, please contact our billing department immediately to set up payment arrangements. Billing fees will be added each month your bill is delinquent.
If you fail to pay what you owe for services, we will be unable to continue providing medical care and your account will be turned over to a collection agency.
PAPERWORK:
Please allow 72 hours for all paperwork, such as physical forms.
There is a $5 charge for additional immunization records.
APPOINTMENTS:
We strive to meet your needs for an appointment as soon as possible. We suggest the following to get an appointment that best fits your schedule and choice of provider:
*Well Child Checks/Physical - this is a longer appointment time and requires advance scheduling, please schedule at least 1-2 months in advance. You may schedule one year in advance. We would like to see your child annually after infancy. Most insurances will only cover well child checks every 365 days. PLEASE BRING YOUR SPORTS PHYSICAL FORM WITH YOU TO YOUR APPOINTMENT!! IF YOU ARE NEW TO US, PLEASE BRING YOUR CHILD'S IMMUNIZATION RECORDS AND YOUR CURRENT INSURANCE CARD!!
*Sick/Acute Visits - we can usually see you the same day you call, please call as early in the day as possible.
If you have multiple concerns to discuss with the provider, be sure to inform the receptionist so we can provide the best possible appointment length.
Please arrive at least 15 minutes before your appointment time to assure time for any paperwork and insurance verification. You may be asked to reschedule your appointment for failure to arrive on time.
If you are unable to make your appointment, please cancel at least 24 hours in advance. Failure to do so may result in a missed appointment fee and/or discharge from the practice.
We encourage continuity of care to ensure the best care for your child. For this reason, we discourage the use of primary providers from other practices while your child is a patient in our practice.
